How to Proceed

Georgina Sight Initiative receives some donor support to subsidize the cost of ophthalmological procedures for patients who may otherwise be unable to access the specialized treatment. We are obliged to assess the validity of requests for financial assistance and, as a policy, we conduct screening of all requests for assistance basing on individual circumstances and need. To enable this screening, applicants are required to disclose certain financial information. All information is held confidentially. Any offers of financial assistance will be based on individual circumstances and also on the availability of funds.

Applicants will be required to provide the documents listed below for each adult family member:

  1. For self-employed – a copy of prior year’s income tax return;
  2. For employed – Copies of the two most recent monthly pay slips;

They also need to complete an application form that may be obtained from Georgina Eye Clinic Surgery located at Muyenga.
