Georgina Sight Initiative receives some donor support to subsidize the cost of ophthalmological procedures for patients who may otherwise be unable to access the specialized treatment. We are obliged to assess the validity of requests for financial assistance and, as a policy, we conduct screening of all requests for assistance basing on individual circumstances and need.

Applicants are required to provide the documents listed below for each adult family member:
* Self-employed: A copy of the prior year’s income tax return
* Employed: Copies of the two most recent monthly pay slips
* Completed Application form available at: Georgina Eye Clinic Surgery, Muyenga branch
